PDF form templates

TakeNote stores the provider PDFs your firm uses and maps their fields to client data. A prepared template appears under Provider Forms and is available when you complete a form for a client.

Before you upload

Use the current, blank version of the provider form. The template must be a PDF and must not contain information for a previous client. Keep the provider's original field labels and page order so advisers can compare the completed output with the source form.

Fillable PDFs give TakeNote existing form fields to detect. TakeNote also analyses flat PDFs, but those templates require a closer review of the detected field positions before they are used with client data.

Upload the template

Open Provider Forms from the app sidebar and select Upload PDF. Choose the blank provider form from your device. TakeNote adds the PDF to the template library and starts field detection.

Each template row shows its field count and setup state. A template marked Set up still needs review or mapping. A template marked Ready is available in the client form workflow.

Set up
Provider Forms lists uploaded templates, detected field counts, and their current setup state.

Review detected fields

Open the uploaded template to review every field TakeNote detects. The review lists the PDF label and field type, including text inputs, checkboxes, and selection controls. Confirm that the list covers the information advisers need to enter on the form.

Detection identifies where information belongs; it does not decide which client record value supplies it. Remove duplicate or accidental fields and retain fields that require adviser input even when no client value exists yet.

Map client data

Select Continue to mapping to open the visual mapping workspace. Choose a field on the PDF preview, then connect it to the corresponding TakeNote data point in the mapping panel. Mappings cover client details, adviser details, meeting information, and values captured from supporting documents.

Use the field label and the provider's instructions together. For example, map a field labelled Client nameto the client's full name rather than a meeting attendee or document contact. Leave a field unmapped when it should always be completed manually.

Make the template ready

Select Save mapping after reviewing the template. TakeNote validates the mapping and returns the template to Provider Forms. Its field count and status show whether it is ready for client use.

Test a new or revised template with a client record before relying on it in a live case. Review the completed PDF for alignment, tick-box behaviour, date formatting, and fields that the adviser must complete manually. Return to the template mapping when the provider publishes a replacement PDF.
